Offer ends April 8, 2025, and is limited to qualifying Prospective Customers and Current Customers who participate in a Weave product demonstration or make a qualifying purchase at the Weave event booth at the SOMSA 2025 event. To qualify as a "Prospective Customer" or "Current Customer," the participating individual must be a doctor or office manager within Weave’s criteria. Whether an individual qualifies as a Prospective Customer shall be determined by Weave at Weave’s discretion. No purchase of Weave services is required to qualify for the $50 Amazon gift card offer. The gift card will be emailed to the individual participating in the demo within four (4) weeks of completion of the demo. Offer may not be combined with any other offers and is limited to one (1) gift card per office. Offer is not available to current customers or to prospective customers or individuals that have participated in a Weave demo during the prior six (6) months. For Prospective Customers who purchase Weave at the event booth, while supplies last, the Ember Mug 2 will be provided at the booth upon execution of an order for Weave products and services. This offer is not available to current customers, and whether an individual is eligible for the offer shall be determined by Weave at Weave’s discretion. Supplies are limited and will be distributed on a first-come, first-served basis. Recipients are responsible for all taxes and fees associated with receipt and/or use of the gift card or mug, as well as reporting the receipt of these items as required under applicable federal and state laws. Weave is not responsible for and will not replace the gift card or mug if they are lost or damaged, are not used within any applicable timeframe, or are misused by the recipient. Weave is not responsible for any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from the recipient’s participation in the promotion or receipt or use of the gift card or mug. Recipient agrees to indemnify, defend, and hold harmless Weave from and against any and all claims, expenses, and liabilities (including reasonable attorneys’ fees) arising out of or relating to a recipient’s participation in the promotion and/or recipient’s acceptance, use, or misuse of the gift card or mug.
