Weave Healthcare Survey Drawing Terms & Conditions Eligibility:

This drawing is open to individuals who are legal residents of the United States and Canada, 18 years of age or older, who received the survey request directly from Weave, and who have completed the Weave survey during the period of November 17, 2025 at 8 AM MT to December 31 at 11:00 PM (the "Survey Period").
Employees, officers, directors, and agents of Weave and their immediate family members (spouse, parent, child, sibling, and their respective spouses, regardless of where they reside) and those living in the same household, whether or not related, are not eligible to participate.
Void where prohibited by law.

How to Enter:

Participants must complete the Weave survey in its entirety during the Survey Period.
Only one entry per person is permitted.
Completion of the survey constitutes entry into the drawing.
No purchase is necessary to enter or win.

Drawing and Winner Selection:

A random drawing will be conducted on or about January 1, 2026 from all eligible entries received during the Survey Period.
The drawing will be conducted by randomized computer selection. All names will be placed into a computer program and the selection will occur automatically without additional human intervention.
The odds of winning depend on the number of eligible entries received.
The winner will be notified via email within 3 business days of the drawing.

Prize:

The prize is an Amazon gift card.
The approximate retail value of the prize is $50.
The prize is non-transferable and no substitution will be made except as provided herein at the Company’s sole discretion. The Company reserves the right to substitute a prize for one of equal or greater value if the designated prize should become unavailable for any reason.
Winner is responsible for all applicable federal, state, and local taxes and any other costs or expenses associated with prize acceptance and use not specified herein as being provided.

Winner Notification and Prize Claim:

The potential winner must respond to the notification within seven (7) business days to claim the prize.
If the potential winner fails to respond within the specified time, or if the notification is returned as undeliverable, the prize may be forfeited and an alternate winner may be selected.
The potential winner may be required to sign and return an affidavit of eligibility, a liability release, and a publicity release (where permitted by law) within seven (7) days of notification.
Failure to comply with these requirements may result in prize forfeiture.

Publicity:

By participating, participants grant Weave the right to use their name, likeness, and survey responses for promotional purposes and for business development purposes without further compensation, unless prohibited by law.

General Conditions:

Weave reserves the right to cancel, suspend, or modify the drawing, or any part of it, if any fraud, technical failures, or any other factor beyond Weave's reasonable control impairs the integrity or proper functioning of the drawing, as determined by Weave in its sole discretion.
Weave is not responsible for lost, late, incomplete, invalid, unintelligible, or misdirected entries.
By participating, participants agree to release and hold harmless Weave, its officers, directors, employees, and agents from any and all liability for any injuries, losses, or damages of any kind arising from or in connection with the drawing or acceptance, possession, or use of the prize.
All decisions of Weave regarding the drawing are final and binding.

Governing Law:

This drawing is governed by the laws of the State of Utah, without respect to conflict of law doctrines.

Information collected from participants is subject to Weave’s privacy policy, which can be found at https://www.getweave.com/legal/privacy/.
