Eligibility
Offer available only for responses submitted between April 1, 2024 and December 31, 2024, and is only for Weave customers who complete this survey in its entirety. No purchase of Weave services is required. Weave reserves the right to disqualify any response which does not meet quality standards.
Participation
Any information submitted through the web form and/or discussed in a subsequent phone interview may be used for marketing and promotional purposes. Submission of the web form does not entitle you to the $100 Amazon Gift Card. Weave will select, in its sole discretion, those individuals or offices that are eligible for a subsequent phone interview. Only those selected for a phone interview will be eligible for the Amazon gift card. Weave will not provide an explanation for those who are or are not selected to be interviewed. In order to receive the Amazon gift card, you must complete a phone interview with a representative from Weave.
Incentive
After participation in a phone interview, you will receive a $100 Amazon.com Gift Card. The gift card will be emailed to the company participating in the survey, at the email address provided in the web form, within twenty (20) business days of completion of the phone interview
General
Offer may not be combined with any other offers and is limited to one (1) gift card per office. Recipient is responsible for all taxes and fees associated with receipt and/or use of the gift card as well as reporting the receipt of the gift card as required under applicable federal and state laws. Weave is not responsible for and will not replace the gift card if it is lost or damaged, is not used within any applicable timeframe, or is misused by the recipient.
