Prime Days Promotion

1.  Sponsor

This promotion (“Promotion”) is sponsored by Weave Communications, Inc., 1331 N. 1075 W, Suite 300, Lehi, Utah 84043 (“Weave”). This Promotion is in no way sponsored, endorsed, or administered by, or associated with Amazon.com, Inc. or its affiliates. Any questions or complaints regarding the Promotion should be directed to Weave, not Amazon.

2.  Eligibility

This Promotion is open to legal residents of the fifty (50) United States and the District of Columbia who: (a) are at least 18 years of age at the time of participation; (b) are prospective customers of Weave who have not previously purchased or subscribed to any Weave services; and (c) have not participated in a Weave product demonstration within the six (6) months preceding their scheduled demo. Employees, officers, and directors of Weave and its affiliates, subsidiaries, advertising and promotion agencies, and members of their immediate families (spouse, parent, child, sibling, and their respective spouses) or households are not eligible. Void where prohibited by law.

3.  Promotion Period

The Promotion begins on the date of first public announcement and ends on June 19, 2026 (“Offer End Date”). To qualify, a participant must (a) schedule a Weave services demonstration on or before June 19, 2026, and (b) complete the demonstration on or before Juzly 15, 2026 (“Participation Deadline”). Demonstrations completed after the Participation Deadline are not eligible for a gift card.

4.  How to Participate; No Purchase Necessary

No purchase is necessary to participate in or receive a gift card through this Promotion. To participate, eligible individuals must: (a) schedule a live Weave services demonstration with an authorized Weave representative during the Promotion Period; and (b) complete the full demonstration on or before the Participation Deadline.

ALTERNATE METHOD OF ENTRY: To participate without scheduling a demo, send a handwritten postcard with your full name, business name, mailing address, phone number, and email address to: Weave Demo Promotion, 1331 N. 1075 W, Suite 300, Lehi, Utah 84043. Postcards must be postmarked by June 19, 2026 and received by June 26, 2026. Limit one (1) postcard entry per individual. Mechanically reproduced entries are void. Alternate method of entry participants will receive the minimum gift card value ($50). Weave is not responsible for lost, late, misdirected, or incomplete mail entries.

5.  Gift Card Award

Each eligible participant who completes a qualifying demonstration will receive one (1) Amazon.com gift card with a value determined as follows:

1. $50 Amazon.com gift card for completing a standard product demonstration.
2. $150 Amazon.com gift card for offices that operate in the Dental, Optometry, Vet, and Medical verticals and have a Practice Management Solution that supports an integration with Weave’s software.

Gift card amounts are determined by Weave at the time of scheduling and will be communicated to the participant prior to the demonstration. Limit one (1) gift card per business office location (determined by unique physical business address). Gift cards are subject to Amazon.com’s terms and conditions, available at amazon.com/giftcardterms. Weave will use reasonable efforts to mail gift cards to the address provided by the participant within four (4) weeks of the completed demonstration. Weave is not responsible for undeliverable mail resulting from inaccurate or incomplete address information provided by the participant.

6.  Gift Card Terms; Lost or Damaged Cards

Gift cards are issued by Amazon.com and are subject to Amazon.com’s terms and conditions. Weave will not replace any gift card that is: (a) lost, stolen, or damaged after delivery to the address provided by the participant; (b) not used within any applicable redemption period specified by Amazon.com; or (c) misused by the recipient. Participants are encouraged to retain proof of their mailing address and to promptly notify Weave of any address changes prior to gift card mailing.

7.  Tax Obligations

The recipient is solely responsible for all federal, state, and local taxes and fees associated with the receipt and/or use of any gift card. Recipients receiving gift cards with an aggregate value of $600 or more in a calendar year from Weave may be issued an IRS Form 1099-MISC or other applicable tax form. Weave reserves the right to require recipients to complete required tax documentation as a condition of receiving a gift card. Recipients should consult a qualified tax advisor regarding their individual tax obligations.

8.  Offer Restrictions

This Promotion may not be combined with any other Weave offer, promotion, or discount. This Promotion is limited to prospective customers and is not available to existing Weave customers or to any individual who has previously purchased or subscribed to Weave services. Weave reserves the right to disqualify any participant who, in Weave’s sole determination, has attempted to manipulate or abuse the Promotion, submitted fraudulent registrations, or otherwise violated these Terms and Conditions.

9.  Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, WEAVE AND ITS AFFILIATES, OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR: (A) ANY INJURY, LOSS, OR DAMAGE OF ANY KIND, INCLUDING DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, ARISING OUT OF OR IN CONNECTION WITH PARTICIPATION IN THIS PROMOTION OR THE RECEIPT, USE, OR MISUSE OF ANY GIFT CARD; OR (B) ANY PRINTING, TYPOGRAPHICAL, ADMINISTRATIVE, OR TECHNOLOGICAL ERRORS IN MATERIALS ASSOCIATED WITH THIS PROMOTION.

Some jurisdictions do not allow the exclusion or limitation of liability for certain damages, so the above limitations may not apply to you.

10.  Indemnification

By participating in this Promotion, each participant agrees to indemnify, defend, and hold harmless Weave and its affiliates, officers, directors, employees, agents, successors, and assigns from and against any and all claims, losses, damages, expenses, and liabilities (including reasonable attorneys’ fees) arising out of or relating to: (a) the participant’s participation in this Promotion; (b) the participant’s acceptance, use, or misuse of any gift card; or (c) the participant’s breach of these Terms and Conditions. This indemnification obligation shall not apply to claims arising solely from Weave’s own gross negligence or willful misconduct.

11.  Dispute Resolution; Governing Law

These Terms and Conditions and any disputes arising out of or relating to this Promotion shall be governed by the laws of the State of Utah, without regard to its conflict of laws principles. Any dispute that cannot be resolved informally shall be submitted to binding individual arbitration in Salt Lake County, Utah under the rules of the American Arbitration Association, and judgment upon the award may be entered in any court of competent jurisdiction. EACH PARTY WAIVES ANY RIGHT TO PARTICIPATE IN A CLASS ACTION LAWSUIT OR CLASS-WIDE ARBITRATION. Nothing in this section shall limit Weave’s ability to seek injunctive or other equitable relief in any court of competent jurisdiction.

12.  Modification and Termination

Weave reserves the right, in its sole discretion, to cancel, modify, or suspend this Promotion at any time without prior notice if fraud, technical failures, or any other factor beyond Weave’s reasonable control impairs the integrity or proper functioning of the Promotion. Weave reserves the right to disqualify any individual who tampers with the entry process or violates these Terms and Conditions.

13.  Void Where Prohibited

This Promotion is void where prohibited or restricted by law. It is the participant’s responsibility to determine whether this Promotion is lawful in their jurisdiction before participating.

14.  Privacy

Personal information collected in connection with this Promotion will be used by Weave in accordance with its Privacy Policy, available at weave.com/privacy-policy. By participating, participants consent to the use of their personal information as described in the Privacy Policy, including for purposes of administering the Promotion and delivering gift cards.
