Letter of Authorization (LOA)
I verify that I am, or represent, the below-named customer, authorized to change the primary carrier for the telephone numbers listed. The name and address provided is the name and address on record with my current carrier for each telephone number listed. I authorize Weave to act as my agent in my behalf and notify my current carrier to change my preferred carrier for the listed numbers, and to obtain any information Weave deems necessary to make the carrier change, including, for example, customer name and number, billing records, service records and network and equipment records. I understand that only one carrier may be designated as the interstate or interLATA preferred interexchange carrier for any one number. I also understand that I may consult with the carrier as to whether a fee will apply to the change in preferred carrier. This LOA will become effective on signature date and will remain in effect unless revoked in writing prior to that date.
