1. Overview

This document outlines the terms and conditions for the "Decision Maker Demo Referral Incentive Promotion" (the "Promotion"). The Promotion is designed to reward individuals who facilitate an introduction between a Sales Representative of Weave Communications, Inc. (“Weave”) and a Decision Maker at their or another organization, resulting in a completed product demonstration.

2. Eligibility

Participation in the Promotion is open to any individual (the "Referrer") who is engaged in a direct, in-person interaction with a Weave Sales Representative. The Decision Maker (the "Referred Party") must be an individual within the organization who has the authority to approve the purchase of the products or services being demonstrated by Weave. Employees of Weave and their immediate family members are not eligible to participate as a Referrer. Offer is not available to current customers or to prospective customers or individuals that have participated in a Weave demo during the prior six (6) months.

3. Promotion Mechanism and Award

A. Qualifying Referral A Qualifying Referral occurs when the Referrer, during an in-person interaction, or later via written communication with a Sales Representative, refers the Sales Representative to a specific Decision Maker, and the following conditions are met:

The Sales Representative successfully connects with the Referred Party.
The Referred Party agrees to and participates in a product demonstration (the "Demo").
The Demo is successfully completed.

No purchase is required.

B. Award Criteria and Timing The Referrer will be eligible to receive a one-time payment of $500.00 USD (the "Incentive") in the form of a gift card upon the successful completion of the Demo, provided that:

The Demo must be completed within thirty (30) calendar days of the date the Sales Representative receives the referral from the Referrer.
The Incentive will be processed and awarded within four to six weeks following the confirmed completion of the Demo.

4. General Conditions

Single Payment: Only one Incentive payment will be awarded per unique Decision Maker/organization combination, regardless of how many products are demonstrated or how many times the Referrer provides the same referral.
Tax Liability: The Referrer is responsible for all applicable taxes and reporting obligations related to the receipt of the Incentive. Weave may issue a 1099 or equivalent tax document as required by law.
Verification: Weave reserves the right to verify the identity of the Referrer and the Decision Maker and the validity of the referral.
Right to Modify or Terminate: Weave reserves the right to modify, suspend, or terminate this Promotion, the Incentive, or these Terms and Conditions at any time and for any reason without prior notice.

5. Contact and Submission

The tracking of all referrals and Demo completion dates will be managed by Weave personnel. The official record of the referral date will be the date entered by the Sales Representative into the official CRM system.

6. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of Utah without regard to its conflict of law principles. Any disputes related to this Promotion shall be resolved in the courts located in Utah. This is a limited-time promotional offer. By participating, the Referrer agrees to these Terms and Conditions.
